About Through Age Disability Team:

Ceredigion Disability team is a Specialist Team working across Ceredigion with adults and children with disabilities.

The team comprises social workers, social work assistants and support workers we also work closely with health colleagues at Hywel Dda University Health Board to provide support, advice and therapeutic intervention to Adults and children living in Ceredigion who have a disability.

The Team aims to support you or your relative in maximising their independence, security, and well-being wherever possible in their own community.

The Team works with adults and children with disabilities and/or Autism and where the impact of their disability is

  • Substantial
  • Long term
  • Impacting upon their ability to carry out normal day to day activities

A member of the team will carry out a needs assessment, which involves the following:

  • Looking at your circumstances
  • Assessing the personal outcomes, you want to achieve
  • Looking at the barriers you have to achieving your identified outcomes
  • Identifying any risks to you if your personal outcomes are not achieved
  • Identifying your strengths and capabilities

The assessment will be a conversation between you and/or your family and the social care officer to work out how to meet your care and support needs. The assessment must identify solutions and how they will be delivered. All five of the elements listed above must be considered in the assessment, and from this, a decision will be reached about which needs are eligible to receive care and support from the Disability Service.
